Retina-Vitreous Surgery
Problems with the retina and vitreous can lead to vision loss and blindness. Surgery can correct problems before vision is lost or prevent further deterioration from occurring.
The vitreous is the clear, gel-like substance that makes up the center of the eye, accounting for approximately two-thirds of the eye's volume, giving it its shape. Because of its large, soft consistency, the vitreous is commonly affected by various diseases that may cause it to cloud, fill with blood or harden, making it difficult for light to properly reach the retina. This may lead to blurred vision, tears or other serious conditions.
Patients with disease or injury to the vitreous may benefit from a vitrectomy. This procedure removes the vitreous by suctioning it out with tiny instruments that are inserted into the eye. After removal, your doctor may treat the retina with a laser, cut or remove scar tissue, flatten detached areas of the retina, or repair holes or tears in the retina. Patients may experience mild discomfort and redness for several days after this procedure, and often have their eye patched for the first day.
Although results vary depending on the individual condition treated, most patients experience improved visual acuity after this procedure. Vitrectomy is most effective in treating conditions such as macular hole, retinal detachment, diabetic retinopathy, vitreous hemorrhage or an injury or infection in the vitreous.
Although this procedure is considered safe, there are certain risks associated with any surgical procedure. Some of these risks include retinal detachment, fluid buildup, growth of new blood vessels, infection and further bleeding into the vitreous gel. Patients can minimize these risks by choosing an experienced doctor to perform their procedure.

Diabetic Retinopathy
Although damage caused by diabetic retinopathy cannot be corrected, patients diagnosed with the condition can be treated to slow its progression and prevent further vision loss. Treatment modalities include laser and surgical procedures.
Diabetic retinopathy is the most common type of diabetic eye disease and the leading cause of blindness in the US. This condition is caused by blood vessel changes within the retina that lead to swelling and leaking of fluid. It can also cause growth of abnormal new blood vessels on the surface of the retina. There are four stages of diabetic retinopathy that begin with the occurrence of microaneurysms and eventually lead to abnormal blood vessels on the surface of the retina that can easily leak fluid and cause severe vision loss and even blindness.
Treatment for early stages of diabetic retinopathy and other condition usually focuses on maintaining levels of blood sugar, blood pressure and blood cholesterol, in order to prevent any permanent damage from occurring. For more advanced stages of the condition, laser surgery is often effective in shrinking the abnormal blood vessels through over 1,000 laser burns in the area of the retina. This procedure, known as scatter laser treatment, usually requires two or more sessions in order to fully remove the blood vessels.
The fluid can also leak into the center of the macula and cause swelling and blurred vision, a condition known as macular edema. The risk of developing macular edema increases as diabetic retinopathy progresses.
Macular edema can also be treated through a laser procedure, called focal laser treatment, which places hundreds of laser burns in the area of retinal leakage to reduce the amount of fluid in the retina. This treatment only requires one treatment.
Laser treatments can be performed in your doctor's office and are performed with anesthetic eye drops to minimize pain during the procedure. Patients may experience blurry vision for the rest of the day, and should rest at home. It is important for patients to realize that these procedures cannot cure diabetic eye conditions, but rather help reduce vision loss for patients with advanced cases of the condition.

Diabetic Evaluation and Treatment
Patients with diabetes should have an eye exam every year. Early detection of eye disease -- including diabetic retinopathy, cataracts and glaucoma -- can prevent vision loss and other complications.
A comprehensive eye exam involves a visual acuity test to measure vision at various distances, and a dilated eye exam to examine the structures of the eye for any signs of disease. During this test, your doctor can examine the retina and optic nerve with a special magnifying lens. Tonometry may also be performed during a comprehensive eye exam to measure the pressure inside the eye with a special instrument.
Patients with diabetes are at an increased risk of developing eye diseases that can lead to vision loss and blindness, such as diabetic retinopathy, cataracts and glaucoma. In fact, diabetes is actually the leading cause of blindness in the United States.
Diabetic eye conditions often develop without any noticeable vision loss or pain, so significant damage may already be done to the eye by the time the patients notices any symptoms. For this reason, it is important for diabetic patients to have their eyes examined at least once a year. Early detection of eye disease can help prevent permanent damage.
Diabetic-related eye problems develop from high blood sugar levels, which can cause damage to blood vessels in the eye. Over 40 percent of diabetic patients will develop some form of eye disease in their life. The risk of developing eye problems can be reduced through regular eye exams and by keeping blood sugar levels under control through a healthy diet and regular exercise.
Diabetic eye conditions develop in the retina as a result of microvascular abnormalities. The tiny blood vessels within the retina develop microaneurysms and begin to leak blood. As new blood vessels develop, they also leak blood and can cause hemorrhages and permanent damage to the retina.
While diabetics struggle with a high sugar count in the blood along with insufficient insulin production, it is actually the lack of oxygen in the blood that causes eye problems and leads to vision loss.
Eye exams should be performed at least once a year or as soon as any potential problems are detected in order to ensure early detection of any serious conditions. Early detection is the strongest protection against diabetic eye diseases.
